Case Management 服务

The 咨询 and 精神健康中心 (CMHC) works directly with students to offer support and case management services.

What is Case Management

Case managers work with students by coordinating intervention and support efforts across campus and community systems and routinely address these areas:

  • Assessing a student's needs and support systems
  • Planning for stressful times and helping clients develop coping mechanisms
  • Providing psychoeducation related to mental health, substance use or other well-being concerns with compassion and care
  • Provide resources on- or off-campus that support the student's treatment goals
  • Coordination of care after a hospital discharge
  • Transfer of care and mental health referrals

Mental health case managers work with students to develop treatment plans addressing their current needs. An assessment of the student's strengths, personal needs and presenting concerns will be conducted on your first appointment, which is typically 45-50 minutes long.

The case manager will gather information to best help you identify those needs. Subsequent appointments will likely be shorter approximately 30 minutes and will follow up on your ongoing goals, 需要和全面护理.

You may schedule an initial appointment with the case manager by contacting CMHC at 208-885-6716,发邮件 CMHC@sandybb.net or simply stopping by at 1210 Blake Ave., in Mary Forney Hall on the 3rd 地板上.

This will vary based on each individual's circumstances. Some may meet with a case manager only once or twice, and other students may require ongoing meetings to ensure needs are being met and all resources are in place.

是的. As with other providers at CMHC, case managers will keep your confidentiality at the forefront of their work with you. Case management records and others resulting from the services you receive are confidential and will not be released to any other person or agency without your written consent.

There may be times you'd like your case manager to coordinate with others on- or off-campus and in these cases, a release of information would be discussed to allow the case manager to work to best serve and support you.

State and Federal laws and/or professional ethics place some limits on confidentiality and may require releasing information, without your permission, to designated authorities. We are required to report or take immediate interventions if you:

  • Report a credible and imminent threat of danger to yourself or another person
  • Report or describe any physical abuse, neglect or sexual abuse of children or vulnerable adults

Additional Case Manager services are offered through the Dean of Students office located in the Teaching and Learning Center, 232房间.
